目的: 分析镍钛-聚髌器结合Herbert螺钉及钢缆治疗髌骨下极粉碎性骨折的手术方法及疗效。

方法: 2008年3月至2010年6月,采用镍钛-聚髌器结合Herbert螺钉及钢缆治疗髌骨下极粉碎性骨折34例,男19例,女15例;年龄22~75岁,平均38.6岁。术后早期功能锻炼,同时定期随访,并对膝关节功能恢复进行评定。

结果: 所有患者获随访,时间7~20个月,平均13.5个月,所有骨折获得解剖复位。疗效评定按照Böstman等临床评分系统评分如下:总分28.43±2.34,其中优(28~30分)21例,良(20~27分)13例,差(<20分)0例。

结论: 镍钛-聚髌器结合Herbert螺钉及钢缆是治疗髌骨下极粉碎性骨折的有效方法,有较好的临床应用价值。

Application of Ni-Ti patellar concentrator combined with Herbert screw and wirerope for the treatment of comminuted patellar inferior pole fractures
ABSTRACT  

Objective: To evaluate the surgical techniques and clinical effect of Ni-Ti patellar concentrator combined with Herbert screw and wirerope in treating comminuted patellar inferior pole fractures.

Methods: From March 2008 to June 2010,34 patients with comminuted patellar inferior pole fracture were treated with Ni-Ti patellar concentrator combined with Herbert screw and wirerope. Of them,there were 19 males and 15 females with an average age of 38.6 years old(ranged from 22 to 75). Early functional exercise was guided postoperatively and the patients were regularly followed up and their outcomes were assessed.

Results: All patients were followed up with an average of 13.5 months (ranged,7 to 20). All fractures healed. According to Böstman Clinical Rating System,the average score was 28.43±2.34,and excellent results in 21 cases,good in 13 cases and no poor case.

Conclusion: Ni Ti patellar concentrator combined with Herbert screw and wirerope for the treatment of comminuted patellar inferior pole fracture is an effective method and of great value to clinical application.
