目的:探讨应用微创内固定结合高强度注射式人工骨植骨治疗股骨远端骨质疏松性骨折疗效。
方法:2005年2月至2008年2月应用微创内固定系统(less invasive stabilizing system,LISS)内固定结合高强度注射式人工骨(MIIGTMX3)植骨治疗股骨远端骨质疏松性骨折26例,男15例,女11例;年龄59~81岁,平均68.5岁;AO分型:A 3型9例,C1型6例,C2型7例,C3型4例。
结果:手术时间平均(110±15) min( 90~135 min).全部病例随访12~28个月,平均18个月。愈合时间平均20.5周( 16.5~28周) .术后无感染、内固定松动、断裂及骨不愈合病例。按照Rasmussen 骨折复位放射评分标准,优19 例,良7 例。术后1 年,关节活动范围2°~125°;HSS膝关节评分平均(86.5±8.2)分( 58~99 分) ,其中优22例,良2 例,中1例,差1 例,优良率92.6%.
结论:微创内固定结合高强度注射式人工骨植骨是治疗股骨远端骨质疏松性骨折的有效方法。 |

Treatment of osteoporotic distal femur fractures with less invasive stabilizing system(LISS) and high strength injecta- ble graft |
|
ABSTRACT
Objective: To evaluate the clinical results of less invasive stabilizing system (LISS) and high strength injectable graft in the treatment of osteoporotic distal femur fractures.
Methods: From Feb.2005 to Feb.2008,26 cases (15 males,11 females) of osteoporotic distal femur fractures were treated with less invasive stabilizing system(LISS) and high strength injectable graft. The mean age of the patients was 68.5 years(ranging from 59 to 81 years). According to AO classification,there were 9 cases type A3,6 cases type C1,7 cases type C2,4 cases type C3.
Results: The mean operation time was (110±15) min(ranging from 90 to 135 min). The patients were followed-up for from 12 to 28 months(averaged 18 months). The mean healing time was 20.5 weeks(ranging from 16.5 to 28 weeks). No deep infection,fixation loosening,breakage or failure of implants were observed after the operations. According to Rasmussen radiological evaluation,the results showed 19 cases for “excellent”,7 cases for “good”. One year postoperatively,the mean ROM of the knees was 2° to 125°,the HSS knee score was from 59 to 99 points with average of (86.5±8.2) points. According to HSS scoring system,the results showed 22 cases for “excellent”,2 cases for “good”,1 for “fair” and 1 for “poor”.
Conclusion: The less invasive stabilizing system(LISS) and high strength injectable graft is an effective way for the treatment of osteoporotic distal femur fractures. |
