目的：采用后侧入路治疗腰椎滑脱症，给予复位并同时进行三柱固定融合，评价其远期疗效。
方法：1999年3月至2007年5月总收治腰椎滑脱症患者184例，采用后路单钉-沟槽柱翼钢板复位固定加椎间融合器(WDFC)进行椎间固定融合。87例使用1枚WDFC椎间固定融合，97例使用2枚WDFC椎间固定融合。随访观察患者术前、术后和随访期间，固定节段的椎体间有无移位、滑脱有无复发、钢板及WDFC有无松动、塌陷、椎间隙高度是否有丢失。
结果：随访8个月～5年9个月，平均23个月。根据Nakai评价标准，优142例，良34例，可8例，优良率95.6%.79个Ⅰ度滑脱椎体术后均完全复位；87个Ⅱ度滑脱椎体中有9个术后留有Ⅰ度滑脱；21个Ⅲ度滑脱椎体中有5个术后留有Ⅰ度滑脱；2个Ⅳ度滑脱椎体中1个留有Ⅰ度滑脱，1个留有Ⅱ度滑脱。随访结果显示，滑脱间隙的高度与术后其高度无明显差异，无滑脱复发，钢板及WDFC均无松动塌陷等。
结论：采用单钉-沟槽柱翼钢板加椎间融合器后路复位三柱固定手术治疗腰椎滑脱症是一种比较可靠的手术方式，很好地复位、维持椎间隙高度和节段稳定，融合率高。减少了术后断钉和复发滑脱等问题。 |

| Treatment of lumbar spondylolisthesis by posterior restoration and three-column fixation |
|
ABSTRACT
Objective: To investigate the late results of using posterior restoration and three-column fixation to treat lumbar spondylolisthesis.
Methods: One hundred and eighty-four patients with lumbar spondylolisthesis were collected from March 1999 to May 2007,they were treated by posterior restoration and fixation with single nail-grooved tail steel plate and fixed with cage(WDFC). Among these cases,87 cases were fixed with one WDFC,97 cases were used two WDFCs.
Results: All patients were followed up for 8 to 69 months(averaged 23 months). According to Nakai standard,the results was excellent in 142 cases,good in 34,fair in 8,the excellent and good rates were 95.6%. Seventy-nine vertebraes withⅠdegree spondylolisthesis were reduced after surgery. Eighty-seven vertebraes withⅡdegree spondylolisthesis were reduced except 9 withⅠdegree spondylolisthesis left. Twenty-one withⅢdegree spondylolisthesis were reduced except 5 withⅠdegree spondylolisthesis left;In 2 withⅣdegree spondylolisthesis,one withⅠdegree spondylolisthesis left and the other withⅡdegree spondylolisthesis left. The follow-up results showed that there was no statistical significance in the height of intervertebral space between preoperation and postoperation,and no recurrence was observed and no single nail-grooved tail steel plate and WDFC were loose or crashed.
Conclusion: Posterior restoration and three-column fixation is a positive modus operandi to treat lumbar spondylolisthesis,which can reduce excellently,keep the height of intervertebral space and stabilization of segment,obtain high rate of fusion,and cut down complication. |
