中文摘要:
目的:探讨全钢丝加压固定治疗髌骨骨折对早期膝关节功能恢复的作用。
方法:髌骨骨折患者21例,男15例,女6例;年龄24～65岁,平均34岁。髌骨粉碎性骨折8例,髌骨上极分离骨折2例,下极分离骨折6例,中份骨折5例。采用自行设计的全钢丝加压固定治疗髌骨骨折的方法,于已复位的髌骨上纵行垂直钻好的2个骨孔内分别穿入3道钢丝,1道U形捆扎,1道向对侧半环形捆扎,两侧对称,另1道为"8"字加压固定。
结果:21例均获随访,随访时间3～15个月,术后4～6周骨折愈合。依照张春才膝关节疗效评定标准:优19例,良2例。
结论:全钢丝加压固定治疗髌骨骨折强度可靠,不需外固定,术后膝关节能早期功能锻炼,避免了膝关节强直等并发症的发生。 |

| Clinical value of whole steel-wire compression fixation for treating patella fracture to early functional recover of the knee |

ABSTRACT
Objective:To investigate whole steel-wire compression fixation for treating patella fracture to recover the function of the knee earlier.
Methods:There were 21 patients with patella fracture involving 15 males and 6 females,with an average age of 34 years(ranged 24 to 65).Among 21 patients,8 patients were comminuted fractures,2 were superior patellar pole diastatic fracture,6 were anus perineum diastatic fracture,5 were mid fracture.The whole steel-wire compression fixation was adopted to treat patella fracture.Three steel-wires were penetrated into two bone hole which had been drilled verticaly respectively.One was bundled up like "U",and one was bundled up from side to side in half a ring symmetrically,another was used to compressed fixate like "8".
Results:Twenty-one cases were followed up for 3 to 15 months,time of fracture healing was 4 to 6 weeks after operation.According to the evaluation criterion of the knee therapeutic effect of ZHANG Chun-cai,the result was excellent in 19 cases,good in 2 cases.
Conclusion:There are characteristic by the reliable intension,no requiring external fixation,and early function exercise of the knee which avoiding complication,such as the knee ankylosis in the whole steel-wire compression fixation for treating patella fracture. |
