目的:探讨克氏针内固定结合人工骨(ACPC)填塞治疗跟骨关节内骨折的治疗方法和效果,为临床适应证的选择提供依据。
方法:85例(95侧)跟骨关节内骨折,男62例,女23例;年龄19～52岁,平均35.5岁。采取经跟距关节外侧弧形切口(Kocher切口),直视下撬拨复位,恢复跟距关节面平整及跟骨外形,行克氏针内固定,跟骨体内空隙以人工骨(ACPC)充分填塞。术后踝关节90°位石膏托固定。
结果:85例平均随访1.5年(6个月～2年),按Kerr足部评分系统评估术后功能,术后平均评分(89.68±10.22)分。其中优70侧,良16侧,可9侧,优良率90.5%(86/95)。
结论:跟骨关节内骨折可尽量采用有限内固定治疗,克氏针内固定结合人工骨(ACPC)填塞治疗跟骨关节内骨折,具有创伤小、操作简单、费用低、并发症少等优点,严格掌握适应证,能取得满意的疗效。 |

| Kirschner wires fixation and ACPC grafting for the treatment of fractures of the calcaneus |

ABSTRACT
Objective: To investigate the cilincal effects of Kirschner wires fixation plus ACPC grafting for the treatment of intraarticular fractures of the calcaneus,and to promote the cilincal application of this surgery.
Methods: Eighty-five patients (95 calcaneus) including 62 male and 23 female were reviewed.The age of them ranged from 19 to 52 years,with an average of 35.5 years.All the patients were treated by surgical method with incision passing talocalcaneal joint (kocher-incision) and Kirschner wires fixation plus compacted ACPC grafting after anatomic reduction of talocalcaneal joint and restore the contour of heel.After operation,the ankle joint was kept in functional position with plaster slab.All of them were followed up,and the duration of follow-up ranged from 6 months to 2 years,with an average of 1.5 years.The results were evaluated by the Maryland foot scoring system.
Results:The excellent result was obtained in 70 patients,good in 16 and fair in 9.The postoperative average score was 89.68±10.22.The excellent and good rate was 90.5%(86/95).
Conclusion: The treatment of intraarticular fractures of the calcaneous with Kirschner wire fixation and compacted ACPC grafting has the advantages of small injuries,better reduction,easier manipulation,less associated complications and lower treating money.Strict indications for operation must be followed to obtain satisfactory therapeutic effects. |
