中文摘要:
目的:探讨关节镜辅助下内固定与切开复位内固定治疗低能量胫骨平台骨折的临床疗效。
方法:45例低能量胫骨平台骨折,治疗组23例采用关节镜监视下闭合或切开复位内固定,其中男13例,女10例;年龄19~64岁,平均44.3岁;SchatzkerⅠ型10例,Ⅱ型7例,Ⅲ型6例。对照组22例行切开复位内固定,其中男14例,女8例;年龄21~65岁,平均45.7岁;SchatzkerⅠ型11例,Ⅱ型2例,Ⅲ型9例。采用HSS膝关节功能评分评价比较两组的临床疗效。
结果:45例随访10~36个月,平均16个月。治疗组疗效优于对照组,且术时X线暴露率治疗组低于对照组,治疗组的Ⅱ期手术率低于对照组。
结论:关节镜监视下复位内固定比切开复位内固定临床疗效好,减少切口感染、创伤性关节炎等并发症,减少了术中X线的暴光率,减少Ⅱ期手术率。 |

| Effect comparison of internal fixation between under arthroscope and open operation for treatment of low energy tibial plateau fracture |

ABSTRACT
Objective:To investigate the clinical effect of internal fixation under arthroscope for the treatment of the low energy tibial plateau fracture.
Methods:Forty-five patients of low energy tibial plateau fracture were divided into two groups.Twenty-three patients of treatment group were fixed under arthroscope including 13 male and 10 female,with an average age of 44.3 years ranging from 19 to 64 years.There were 10 cases in type Ⅰ of Schatzker,7 cases in type Ⅱ,6 cases in type Ⅲ.Twenty-two patients of control group were treated with open internal fixation including 14 male and 8 famale,with an average age of 45.7 years ranging from 21 to 65 years.There were 11 cases in type Ⅰ of schatzker,2 cases in type Ⅱ,9 cases in type Ⅲ.The therapeutic effect of two groups were evaluated by HSS clacification.
Results:All patients were followed up for 10 to 36 months,mean 16 months.The rate of treatment group was significantly better than that of control group.The rate of the explosure to X-ray in the treatment group was lower than that of the control group during the operation.
Conclusion:The clinical effect of internal fixation under arthroscope is better than that of the other group.Operation under arthroscope can reduce the complications such as osteoarthritis,infection in cut and operation again so on. |
