目的:探讨分期石膏固定治疗幼儿先天性髋关节脱位的疗效。
方法:本组97例先天性髋关节脱位患儿（共134髋）中男17例,女80例;年龄0.5~3.5岁。左侧34例,右侧26例,双侧37例。按Zionts脱位程度分级:Ⅰ度9髋,Ⅱ度87髋,Ⅲ度38髋。根据年龄分成4组:1岁以下组（Ⅰ组）9例;1~2岁组（Ⅱ组,含2岁）45例;2~3岁组（Ⅲ组）37例;3岁以上组（Ⅳ组）6例。均行三至四期石膏外固定治疗。采用Mckay临床评定标准和SeverinX线评定标准进行疗效评价。
结果:97例经治疗1年后评价疗效:Mckay临床评分优88髋,良35髋,可7髋,差4髋,Ⅰ组和Ⅱ组优良率100%（13/13,62/62）,Ⅲ组优良率86.3%（44/51）,Ⅳ组优良率50.0%（4/8）,总优良率91.8%（123/134）,差异有显著性统计学意义,P<0.0083;SeverinX线评分优80髋,良40髋,可9髋,差5髋,Ⅰ组和Ⅱ组优良率100%（13/13,62/62）,Ⅲ组优良率82.4%（42/51）,Ⅳ组优良率37.5%（3/8）,总优良率89.6%（120/134）,差异有显著性统计学意义,P<0.0083。
结论:分期石膏外固定是治疗幼儿先天性髋关节脱位的可靠方法,患者应尽早进行治疗。 |

| Plaster external fixation in stages for treatment of congenital dislocation of hip in young children |

ABSTRACT
Objective: To study therapeutic effects of plaster external fixation in stages for treatment of congenital dislocation of hip in young children.
Methods: Ninety-seven patients(134 hips) with congenital dislocation of hip including 17 male and 80 female ranging in age from 0.5 to 3.5 years,were treated with plaster external fixation in stages.Among all 97 patients with congenital dislocation of hip 34 patients were left side dislocation,26 patients were right side and 37 patients were double side.According to Zionts dislocation grades,9 hips were degree Ⅰ,87 hips were degree Ⅱ and 38 hips were degree Ⅲ.All the patients were divided into 4 groups based on age:Group Ⅰ covered 9 patients at age less than 1 year;group Ⅱ covered 45 patients at age from 1 to 2 years(including 2 years);group Ⅲ covered 37 patients at age from 2 to 3 years;group Ⅳ covered(6 patients) at age greater than 3 years.Plaster external fixation in 3-4 stages was performed,and Mckay clinical criteria and Severin′s radiographic criteria were used to evaluate clinical effects.
Results: The therapeutic effect was evaluated after one-year treatment.According to Mckay clinical criteria,88 hips reached excellent results,35 good,7 fair and 4 bad,the excellent and good rate in group Ⅰ,Ⅱ,Ⅲ and Ⅳ was 100%,100%,86.3% and 50.0% respectively(13/13,62/62,44/51,4/8),with total excellent and good rate of 91.8%(123/134).The results among the four groups statistically were different from each other(P<0.0083).The results produced by Severin′s radiographic criteria evaluation were excellent in 80 hips,good in 40 hips,fair in 9 hips and bad in 5 hips,the excellent and good rate in group Ⅰ,Ⅱ,Ⅲ and Ⅳ was 100%,100%,82.4% and 37.5% respectively(13/13,62/62,42/51,3/8),with total excellent and good rate of 89.6%(120/134).The difference between the four groups was statistically significant(P<0.0083).
Conclusion: Plaster external fixation in stages is an effective method for treating congenital dislocation of hip in young children,and the patients should be treated as soon as possible. |
