目的:探讨CT引导下经皮空心钛合金拉力螺钉内固定治疗骶髂关节损伤的临床疗效。

方法:骶髂关节损伤患者36例,其中男26例,女10例;年龄19~68岁,平均35岁。ISS评分平均19分。按Tile分类:B1型6例,B2型16例,B3型4例;C1型4例,C2型3例,C3型3例。入院至手术时间平均5d。采用CT引导下经皮空心钛合金拉力螺钉内固定术治疗,硬膜外麻醉下共置入48枚空心钛合金拉力螺钉。

结果:手术时间30~110min,平均42min;术中失血10~35ml,平均18ml。1例因肺功能障碍延迟手术,1例行开颅术,死于多脏器衰竭。术后随访3~27个月,平均17.9个月。依据骨盆创伤治疗标准,影像学评价优29例,良6例,差1例;临床评价优23例,良13例。并发症包括轻度下腰痛1例、骶尾部痛1例。

结论:CT引导下经皮空心钛合金拉力螺钉内固定治疗骶髂关节损伤,置钉精确,安全有效,并发症少。

CT guided percutaneous cannulated lag screw fixation treating sacroiliac joint trauma
ABSTRACT  

Objective:To explore the clinical results of the percutaneous internal fixation of sacroiliac joint trauma by CT guided technique.

Methods:Thirty-six patients with pelvic inchuded 26 male and 10 female with the average age of 35 years,ranging from 19 to 68 years.The average score of ISS was 19. According to Tile classification,6 patients were in Type B1,16 in B2,4 in B3,4 in C1,3 in C2 and 3 in C3.The average time from admission to operation was 5 days.All patients underwent the percutaneous internal fixation of sacroiliac joint trauma by CT guided technique,48 percutaneous Ti-alloy lag screws were inserted under epidural anesthesia.

Results: The average time of procedure was 42 minutes ranging from 30 to 110 minutes;the average blood loss was 18 ml ranging from 10 to 35 ml during operation.One patient delayed operation due to respiratory difficulties and one underwent craniotomy but postoperative death related to MOF.After a period of 3 to 27 months followed-up,according to effective standard of peliv trauma,the result of radiography was excellent in 29 patients,good in 6 and poor in 1;the clinical evaluation was exellent in 23,good in 13.The complications concluded lower back pain in 1 patient and sacroiliac pain in 1 patient.

Conclusion: CT guided lag screw fixation for treatment of sacroiliac joint injuries minimizes operative procedure with few subsequent complications and allows the patients early mobilization.
