目的:探讨引起跟骨骨折术后伤口并发症的可疑因素。
方法:回顾2000年3月-2003年3月,56例行跟骨骨折切开复位内固定的患者,对其一般情况、延迟手术天数、手术时间、止血带使用时间等可疑因素进行详细记录、分析、总结。56例中男49例,女7例;年龄26～55岁,平均42岁。
结果:56例61个跟骨手术后有9个(14.8%)出现了伤口感染、血肿、切口裂开、足跟坏死、慢性骨髓炎等并发症。坠落高度大于4.5m(P<0.005)、延期手术时间较短(P<0.05)、手术时间较长(P<0.05)及止血带使用时间较长(P<0.01),这些可能是引起并发症的可疑因素。
结论:在骨折后7～10d行手术治疗、缩短手术及止血带时间,可以降低手术后伤口并发症的产生。 |

| Study on high risk factors of postoperative complications in the treatment of calcaneal fracture |

ABSTRACT
Objective:To explore high risk factors contributing to postoperative complications in the treatment of calcaneal fracture.
Methods:56 patients with calcaneal fractures, treated with open reduction and internal fixation from March 2000 to March 2003,were reviewed in the study.Among them,49 were male and 7 were femal,ranging in age from 26 to 55 years(mean 42 years).Such related factors as common conditions,delayed operation times,operation time and tourniquet-using time were recorded and analyzed.
Results:Among 56 patients(61 calcaneus),9 calcaneus(14.8%)had follow complications:wound infection,hematom,disruption of wound,heel necrosis and chronic osteomyelitis.The following risk factors maybe be the causation of postoperative complications:①the height of falling more than 4.5 m (P<0.005);②delayed operation time less than 7 days (P<0.05);③operation time more than 2 h(P<0.05);④tourniquet-using time more than 1.5 h(P<0.01).
Conclusion:If the operation is performed at 7 to 10 days after fracture,and the operation and tourniquet-using time are shortened,the postoperative complications can be decreased. |
