目的:探讨改良经皮克氏针固定治疗桡骨远端骨折的手术方法与疗效。
方法:桡骨远端骨折患者43例,男19例,女24例;年龄14~71岁,平均39岁。采用改良经皮克氏针固定法治疗,骨折复位成功后经皮打入3枚克氏针固定。其一从桡骨远折端桡侧进针,从桡骨远折端尺侧出针,克氏针继续进针穿过尺骨下端;其二从桡骨茎突斜行进针经骨折线从桡骨近折端尺侧出针,克氏针继续进针穿过尺骨;其三从桡骨近折端桡侧斜行进针经骨折线从桡骨远折端尺侧出针。
结果:术后随访6~12个月,43例均获临床愈合,无骨折再移位。按Aro评定标准:优13例,良23例,可5例,差2例。
结论:改良经皮克氏针固定治疗桡骨远端骨折固定牢固,损伤小,有利于骨折愈合和关节功能恢复。 |

| Improved percutaneous fixation of Kirschner wire for treatment of the distal radius fracture |

ABSTRACT
Objective:To explore operative method and evaluate the outcome of improved percutaneous fixation with Kirschner wire for treatment of the distal radius fracture.
Methods:Forty-three patients (19 male and 24 female with average age of 39 years ranging from 14 to 71 years) with the distal radius fracture were treated by improved percutaneous fixation with Kirschner wire after diaplasis.The 1st wire was inserted into radial side of the distal end of fracture and pulled out from the ulna side,and then pierced into the inferior ulna;The 2nd wire was inserted into radial process,crossed the fracture line,and then pulled out from the ulnar side and was pierced again into ulna;The 3rd wire was inserted into the radial side of the proximal end of the radius fracture and pulled out from the ulnar side and then also acrossed the fracture line.
Results:All patients were followed up for from 6 to 12 months.Bone healing was achieved in all patients without re-displacement.According to Aro standard for functional evaluation,the excellent result was achieved in 13 cases,good in 23 cases,fair in 3 cases and poor in 1 case.
Conclusion:This method is characterized by firm fixation and less damage.It is helpful to bone healing and functional recovery of joint. |
