目的:应用新型植骨方法结合内固定治疗上肢骨干骨不连。

方法:本组肱骨、尺桡骨、锁骨骨不连93例(107根),其中肱骨50根,尺桡骨28根,尺骨14根,桡骨11根,锁骨4根。利用天鹅型记忆接骨器提供的良好植骨环境,取自体髂骨植骨或辅以同种异体骨植骨,采用夹板式、播种式、钻孔填塞式植骨及必要时骨折端之间加垫植骨以尽量减少骨短缩或缺损。术后早期进行患肢功能锻炼,定期摄X线片复查。

结果:所有病例平均随访2.25年,植骨均在术后2.5~6.0个月、平均3.8个月全部融合,达到骨板样骨替代,无骨髓炎、骨不连、关节僵直等并发症的发生,关节功能恢复良好。

结论:新型植骨方法结合天鹅型记忆接骨器治疗上肢骨干骨不连,可达到有效、充分、稳定植骨,是治疗上肢骨干骨不连新型而有效的方法。

Treatment of non-union upper limb diaphysis with new-bone-grafting method and memorably-compressive swan-like connector
ABSTRACT  

Objective:To study clinical effects of a new-bone-grafting method combined with memorably-compressive swan-like connector in the treatment of non-union of upper limb diaphysis.

Methods:Ninty-three(107 pieces) patients with bony non-union of humerus,ulna,radius and clavicle were reviewed in the present study.Self-iliac graft or allograft were implanted under the special enviroment provided by swan-like memorably-compressive connector(SMC).Several bone-grafting methods such as splinting,seeding and boring,combined with packing and padding were performed.If necessory,the grafting bone was also put between the fracture ends so as to accelerate bone-healing and avoid bone shortness and defect.This new method was applied to 50 pieces of humerus,28 ulna and radius,14 ulna,11 radius,4 clavicle.The patients exercised early and took X-ray examination periodicly after operation.

Results:All the patients were followed up for an average of 2.25 years.The period of bone re-union ranged from 2.5 to 6.0 months,with an average of 3.8 months.The patients with non-union fractures were healed with lamellar bone without any complications such as osteomyelitis and joint stiffiness,and the joint function was satisfactory.



Conclusion:
This graft implantation combined with SMC is efficient,sufficient and stable,indicating that it will be a new effective method to treat non-union of upper limb disphysis.
