目的:探讨一种治疗胫骨下端粉碎多块骨折的方法并评价其治疗效果。
方法:胫骨下端粉碎多块骨折,骨折远段较短,只可置入1枚带锁髓内钉的患者41例,其中男28例,女13例;年龄19～65岁。所有患者采取置入主钉后,再恢复胫骨骨折块的轴线序列,用钢丝绑扎固定骨折块,最后置入远近端锁钉的方法治疗。
结果:41例手术顺利,术后骨折对位对轴良好。39例骨折愈合,骨折临床愈合时间平均14周。术后随访1～6年,按JohnerWruh评分标准优31例,良8例,差2例。
结论:胫骨骨折远段虽只置入1枚锁钉,再用钢丝绑扎固定骨折块,骨折固定仍属牢靠,术后不需石膏外固定,可早期行患肢康复功能锻炼。 |

| Treatment of comminuted fractures of lower end of the tibia by interlocking intramedullary nail |

ABSTRACT
Objective:To explore a method for treatment of comminuted fractures of tibial and evaluate its clinical Results.
Methods: 41 patients (male,28 cases;female,13 cases;with age ranging from 19 to 65 years)with comminuted fractures of lower end of the tibia were treated with interlocking intramedullary nail.The distal segment of fractures was quite short and only one nail could be placed.Operations procedures as:①Place in the main nail;②Restore the axial sequence of fractures;③Fixed fractures with steel wire;④Place in the interlock nails in proximal and distal segment of the tibial fracture.
Results::All operations were successful.Of them,39 cases have been healed with the average of the healing time was 14 weeks.All patients were followed up from 1 to 6 years,according to standard of Johner-Wruh’ score:31 cases excellent,8 cases good,2 cases poor.
Conclusion:Though there is only one interlock nail was placed in distal segment of the tibial fracture and using steel wire to fix it,it is a reliable fractures fixation.Plaster support was not necessary for external fixation and can allow the patients to do earlier rehabilitative exercise. |
