目的:探讨阔筋膜张肌损伤致腰腿痛的诊疗及其机制,并分析与腰椎间盘突出症的关系。
方法:对387例阔筋膜张肌损伤患者的临床表现、手法治疗部位、发病机制进行总结、分析。本组男207例,女180例;年龄13～76岁,平均32岁;病程2d～10年。全部采用选取“治疗灶”为主要治疗部位的手法治疗。
结果:本组随访6个月～3年,治愈329例,占85%;好转50例,占13%;无效8例,占2%。
结论:阔筋膜张肌损伤后引起的功能障碍可代偿性地导致脊柱生物力的平衡失调,并引起腰椎及骨盆周围的肌肉、血管、神经受到牵拉、挤压,出现一系列局部运动、感觉、循环障碍症状,且发现这些症状与L4,5椎间盘突出症的临床表现基本相同。 |

| Clinical research on the lumbocrural pain caused by the injury of tensor fasciae latae treated by manipulation |

ABSTRACT
Objective:To study the diagnosis,treatment and mechanism of lumbocrural pain caused by the injury of tensor fasciae latae,and then analyze the relation between the injury of tensor fasciae latae and lumbar disc herniation.
Methods:We observed the clinical signs and symptoms of 387 patients（male 207 and female 180）and treated them by manipulation,then analyzed the mechanism of such kind of disease.The ages of the patients ranged from 13 to 76,with an average of 32.The duration of illness was 2 days to 10 years.
Results:The effective rate was 98 percent,and the clinical signs and symptoms of the lumbar disc herniation and the injury of tensor fasciae latae were very similar.
Conclusion:The injury of tensor fasciae latae can lead to imbalance of spinal biomechanics,then influence the muscles,blood vessels and pelvis,and then result in such kinds of symptoms basically like the disfunctions of motion,sensation and blood circulation of intervertebral disc herniation of L4,5. |
