高凝、低纤和高脂血症在实验性激素性股骨头坏死发生中的作用

目的:探讨高凝、低纤和高脂血症在激素性股骨头坏死发生中的作用。

方法:对18只新西兰大白兔静脉注射大肠杆菌内毒素(LPS)2次,两次间隔24小时。并在最后一次LPS注射后,臀肌注射3次甲基强的松龙(MP),间隔24小时。对每次药物注射前后12小时、1周、2周、3周和4周进行甘油三脂(TG)、胆固醇(Tch)、高密度脂蛋白(HDL)、部分凝血活酶时间(APTT)、纤溶酶(PLG)、抗凝血酶Ⅲ(ATⅢ)、抗纤溶酶(ATPL)进行测定。

结果:单次注射LPS和联合运用LPS+MP均能引起血液的高凝、低纤和高脂血症,软骨下骨血管中有大量的脂栓和纤维蛋白栓子存在,股骨头中出现典型的骨坏死,并且这些血液学指标之间存在相关性。

结论:LPS和MP均能引起血液的高凝、低纤和高脂血症,共同促进股骨头脂栓和纤维蛋白栓子的形成,引起股骨头微血管的损害和血供的中断,这可能是本模型发生的重要病理学基础。

The influence of hypertriglyceridaemia,hypercoagulability and hypofibrinolysis on experimental femoral head necrosis(FHN)induced by corticoid
ABSTRACT  

Objective:To study the effect of hypertriglyceridaemia,hypercoagulability and hypofibrinolysis on experimental femoral head necrosis(FHN)induced by corticoid

Methods:18 New Zealand rabbits were injected intravenously with LPS twice every 24 hours.After the second injection,MP was given IM three times daily.12 hours before and after each injection,TG,Tch,HDL,APTT,PLG,AT Ⅲ and ATPL were detected;the same detections were done at 1,2,3,4 weeks after the last injection.

Results:Hypertriglyceridaemia,hypercoagulability and hypofibrinolysis appeared after LPS were given.When LPS was given together with MP,the effect on the above mentioned Results was intensified,and there was correlation in between the parameters.Typical osteonecrosis occurred in the femoral head.A large amount of fatty embolisms and thrombosis were detected in the subchondrol venous channels.

Conclusion:Experimental FHN could be produced when hormone was added to Shwartzman reaction.Both LPS and MP could bring serum hypertriglyceridaemia,hypercoagulability and hypofibrinolysis,and there was obvious correlation in between them.Though the changes in the blood was instantaneous,yet fatty embolism and thrombus could be formed,and the blood vessels were damaged giving interruption of blood flow,which then caused the main pathological changes.
