中文摘要:
目的：探讨减少在矫治重度麻痹性屈膝畸形中常见的关节僵直、截骨端错位、神经损伤并发症发生的方法。
方法：应用膝关节加压器对屈膝畸形>40°者37例进行膝后软组织松解、髁上V型截骨、延迟缓慢矫正其屈曲度,早期膝关节功能锻炼。
结果：截骨端平均愈合时间9周,无一例发生错位及神经麻痹症状,远期疗效评价优良率为70%.
结论：本术式是重度麻痹性屈膝畸形较安全有效的矫治方法,它减少了神经牵拉伤及截骨端错位的发生率,有利于膝关节早期功能锻炼及截骨端的愈合 |

| Corrective therapy of severe genuflex deformity with compressive fixation apparatus of knee joint |

ABSTRACT
Objective:To improve the methods of severe genu flexion deformity in order to decrease the incidence of complications such as ankylosis,malposition of osteotomy ends and nerve damage during the surgical corrective procedure
Methods:37 cases of genu flexion deformity(>40°) were treated with compressive fixation apparatus of knee joint.Release of posterior soft tissues of knee joint and supracondylar "V" shape osteotomy were performed to slowly correct the range of flexion of knee joint.Functional exercises of knee joint started at early stage after operation.
Results:The average union time of osteotomy ends was 9 weeks and no complications of malposition and nerve parlysis occurred.According to long-term curative effect evaluation,the excellent and good rate of 70% was obtained.
Conclusion:This is an effective and safe corrective method for the treatment of severe paralytic genu flexion deformity,which has advantages of a reduced incidence of traction injury of nerves and malposition of osteotomy ends,providing conveniences for functional exercises of knee joint at early stage and promoting bony healing. |
