目的:探讨激素性股骨头坏死的发病机理。
方法:22只雄性家兔随机分为两组,正常对照组10只,模型组12只,以肌注糖皮质激素诱导家兔股骨头坏死模型为对象,动态观察下丘脑垂体肾上腺皮质轴(HPA)形态和功能变化,以及股骨头形态学变化。
结果:激素性股骨头坏死模型其HPA轴从形态到功能出现抑制状态,表现为血浆皮质醇(CORT)、促肾上腺皮质激素(ACTH)降低,垂体、肾上腺明显萎缩,股骨头陷窝内骨细胞坏死。
结论:HPA轴从形态到功能的全面抑制状态,可能是激素性股骨头坏死早期发病的直接或间接的原因之一。 |

Experimental study of pathogenesis of steroid induced avascular necrosis of femoral head |

ABSTRACT
Objective To investigate pathogenesis of steroid induced avascular necrosis of femoral head(ANFH)
Methods The ANFH models were produced in 22 male rabbits by injecting intramuscularly glucocorticosteroid and the animals were divided into two groups:10 rabbits served as control group and 12 as the experimental group.The morphological and functional change of hypothalamic-pituitary-adrenal(HPA)axis and the morphologica change of femoral head were observed in the experiment.
Results The morphology and function of HPA axis were observed in experimental group;there were marked decrease of plasma cortisol and adrenocorticotropic hormone,atrophy of hypophysis and adrenal,osteocyte necrosis in femoral head lacuna.These were signs of inhibition of the function of HPA axis.
Conclusion The inhibition effect on the morphology and function of HPA axis may be one of important pathogensis of steroid induced avascular necrosis of femoral head. |
