目的:探讨改良AO张力带与镍钛-聚髌器(NT-PC)治疗髌骨骨折的最佳适应症。

方法:采用张力带法治疗48例,NT-PC法治疗37例。比较两法治疗各型髌骨骨折的疗效及各自的优缺点。

结果:对横型骨折、髌底骨折及髌体粉碎型骨折,两法疗效无显着性差异,张力带法具有简单、体内异物小、不良反应少等优点;对髌尖骨折及全髌粉碎型骨折,NTPC法疗效显着优于张力带法(P<0.05).

结论:对横型骨折、髌底骨折及髌体粉碎型骨折,张力带法应视为有效、可靠的方法而列为首选;对髌尖骨折及全髌粉碎型骨折,NT-PC为最佳选择。

A Comparison Between Two Internal Fixation Methods for the Treatment of Patellar Fracture
ABSTRACT  

ojective:To study the optimum indications of improved AO tension band method and nickel titanium patellar concentrator(NT PC).

Method:48 cases were treated with AO tension band and 37 cases were treated with NT PC.The advantages and disadvantages of the two methods in treating various types of patellar fracture were compared.

Results: There was no significant difference between the effects of the two methods as to transverse fracture,patellar bottom fracture,comminuted fracture of patellar body,while tension band method had the advantages of simplicity,small metallic implant matter in body and few adverse reaction,etc.For patellar tip fracture and whole patellar comminuted fracture,NT PC was significantly better than tension band (P<0 05).

Conclusion: Tension band method should be the first effective and reliable choice for transverse fracture,comminuted fracture of patellar bottom and body,while NT PC is the best choice for patellar tip fracture and whole patellar comminuted fracture.
