中文摘要:
目的:探讨复发性腰椎间盘突出症的手术方法。
方法:采用原切口显露切除椎板残端进入椎管14例,半椎板切除6例。术中摘除髓核,松解粘连,疑有侧隐窝狭窄者12例扩大神经根管并切除膨出的纤维环。没有切除瘢痕粘连组织。
结果:经平均62年随访,优9例,良8例,可2例,差1例。优良率为85%,全部病例均较术前有进步
结论:原切口显露可以有效去除病因,解除症状,对脊柱稳定有利。侧隐窝狭窄是致病的重要原因,多继发于椎间盘突出。瘢痕粘连组织无需切除。

| Modification of the Surgical Approach for Revision of Lumbar Disc Herniation |

ABSTRACT
Objective:To describe a modified approach for revisional operation of intervertebral lumbar disc herniation (LDH)
Methods:The spinal canal was entered through the original incision,exposing the remnants of the resected lamina in 14 cases of recurrent LDH The nucleus polposus was resected and the adhesions released The nerve roots orifices were enlarged and the protruded fibrosus pulposus were resected in 12 cases suspicious of stenosis of the lateral recess The scar tissue was left alone
Results:The follow up period lasted in an average of 6 2 years 9 cases had excellent results,8 good,2 fair and one no change.
Conclusion:Surgical appro ach through the original incision is helpful to eliminate the cause of the recurrent lesions and also beneficial for spinal stability Stenosis of the lateral recess played the chief role in the pathogenesis and during the revision procedure the scar tissue can be left alone. |
